During the first visit, we make sure to obtain important background information, like your medical history, and give you time to get to know your doctor.  We accept most insurances. Facemasks are still required at our office. If you forget your facemask, we have masks available at our office for a $2 charge (until further notice).

Please arrive 15 minutes early, so that we can complete your initial paperwork. Also, please bring the following, along with your completed forms at the time of your appointment:

  • Patient's insurance cards
  • A valid picture ID
  • List of current prescriptions and/or over-the-counter medication, including dose and frequency
  • Information about patient's medical and surgical history
  • Recent test results, x-rays, or relevant records

Insurance and Payment Information

You are responsible for any co-payments and co-insurances at the time of service that are not covered by your insurance. Our office accepts Medicare, major health insurance plans, and a variety of HMOs and PPOs. If your services are private fees, we also welcome private pay. If you have questions regarding billing or further questions on what insurance plans we accept, please feel free to call our office. Questions regarding insurance coverage and benefits should be directed to your employer or insurance company.
